Transition to Adult Services

These clinics are attended by Paediatric Consultant, PDSN, Paediatric Dietitian, adult
 Consultant and a Diabetes Specialist Nurse (DSN.
Transisiton clinics are for young adults aged between 16-19 years. 

Patients are invited to attend the transition clinic by their paediatrician once they have reached their 16th birthday. They are early evening clinics runnning from 3:30pm onwards, so as not to interfere with college attendance.

Transition Clinics in the County - Currently Under Review

 

Young Adult Clinics in Leicestershire and Rutland - Dr Gregory
Every 5th Wednesday afternoon of the month at Coalville. Contact: 01530 467445
 

Patients attending clinics in Leicestershire and Rutland Counties are transitioned via a letter to the adult clinician after the patients reach the age of 16 years. This is service is currently under review and plans are being developed to enhance this service with locality clinics and a adult/paediatric transition nurse appointment.

Aims of the clinic




We aim to provide a service for all your needs at this particular time in your life which may be a little difficult for both you and your diabetes.

 So if you have anything you would like to discuss just get in touch we are only a phone call away!
 

Who Are They For?
 
The clinic usually sees young adults transferred between the ages of 16 years and 18 years up to the age of 22 years when you will step up to an adult clinic. Patients are welcome to bring along a family member, friend or partner to clinic appointments.
 
Who runs the clinic?


It is a joint clinic between the Paediatric and adult service with:

•   Dr James Greening (Paediatric Consultant)
•   Dr Lawrence (Diabetes Consultant)
•   Prof Davies (Diabetes Consultant)
•   Dawn Kitchener (Diabetes Specialist Nurse)
 
Where are the clincs Held?



The clinic is held in the Victoria Building in the Department of Diabetes and Endocrinology Where to find us (See our map) and you will be seen 2- 3 times a year unless you have problems.



What if I can't attend the clinic and I need some advice?

  • Every Monday morning at Diabetes and Endocrinology department, Victoria building LRI If you would like to quit and need help and support. Contact: 0116 295 4141 for an appointment.

 What happens next?

 

Once the young person is 20 years of age and ready to be transferred to the young adult clinic,
they are fully transferred to adult services.
